Terms & Conditions
Business Name: Staff Health
By booking an appointment with Staffs Health, you agree to the following terms and
conditions.

Appointments
Appointments must be booked in advance through the approved booking system or directly
with the practitioner.
Please ensure all personal and medical information provided is accurate and up to date.

Payments
A 50% deposit is due at the time of booking.
Payment of the balance is due at the time of the appointment unless otherwise agreed.
Packages must be paid in full at the time of booking.

Medical Information
Clients are responsible for providing accurate and complete medical information relevant to
treatment.
Failure to disclose relevant medical conditions or injuries may affect treatment outcomes and
safety.

Consent to Treatment
By attending an appointment, you consent to physiotherapy assessment and treatment
provided by Staffs Health.
You may withdraw consent or stop treatment at any time.

Liability

Staffs Health will not be held responsible for:
● Undisclosed medical conditions
● Failure to follow treatment advice or rehabilitation plans
● Loss or damage to personal belongings during appointments
Nothing in these terms excludes liability where it cannot legally be excluded under UK law.

Privacy
Personal information is handled in accordance with our Privacy Policy.
